Hampshire and Isle of Wight Healthcare Foundation Trust has an exciting opportunity for an experienced Nurse to join the Havant Community Care Team. Working in the community, you will provide care to patients with complex care needs, enabling them to receive the care they need in their home.

We are looking for an enthusiastic, caring & motivated professional with excellent clinical, managerial & communication skills, who can work autonomously, supporting the team lead to manage the caseload and staff development.
The post holder must have relevant experience in healthcare and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role. Access to a vehicle and possession of a full driving license is essential.
Main Duties of the Job
Working within professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post holder will be responsible for managing, coordinating, and delegating work ensuring referrals are dealt with according to clinical need. Holistic assessments and care plans are to be recorded in line with Trust and professional record-keeping standards and clinical care at all times must meet acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection, and promotion programmes that improve and reduce inequalities.

Salary: £29,970 to £36,483 a year based on full-time hours.
