The Macmillan SWAN Bereavement Nurses will work: to develop and implement the SWAN Model for End of Life (EoL) and Bereavement Care within BCUHB. to act as a clinical specialist providing information, advice and emotional support to bereaved families. to provide support to ward teams and other staff involved with caring for dying patients and bereaved families. to act as an educational resource for multidisciplinary teams regarding EoL and bereavement care. This will include provision of support and training. The two post holders will be based centrally, working to provide a service across BCUHB.
