Main duties of the job To deliver an advanced level of care to children with complex health needs with or without additional acute conditions through the integration of therapeutic and clinical care. The care is delivered in an unsupervised and autonomous context founded upon expert knowledge bases within the speciality. The aim is to retain children within the family environment or within school, during periods of acute illness/ exacerbation, wherever feasible. Expert advice will be provided to community nurses for children within the locality experiencing acute conditions. To advance an integrated role that co-ordinates an improved continuity and quality of care for patients both in respect to treatment and the patient journey across the primary/ secondary/tertiary care interfaces and other relevant agencies. To participate in risk management assessments and the development ofevidenced based strategies to improve the quality of care both for the individual patient and the target population.
