You will work as part of a team to provide early intervention, prevention and reactive nursing care for people who are homeless or rough sleeping across Bolton including those in hostel or temporary accommodation. As an experienced and skilled nurse, you will demonstrate safe, clinical decision-making and compassionate care, including effective assessment and examination skills as appropriate, for people requiring care. You will use nursing skills and knowledge to provide autonomous holistic assessment, examination and management of patients experiencing homelessness presenting with acute and chronic conditions and/or injuries. You will work in partnership with other healthcare colleagues, voluntary and statutory organisations to ensure that the holistic needs of patients are met wherever possible. When specialist services are required, you will act as the conduit to advocate and support people to access the care required. You will play a vital front line role tackling health inequalities by working with one of the most vulnerable groups in Bolton. Working collaboratively with people and partner agencies, you will proactively engage with individuals in a variety of settings in local communities, helping people to improve their lifestyle and overall health by contributing to a holistic approach that deals with the wider determinants of health. It may be necessary at times to provide street outreach services as part of this role, which will involve offering services for patients in locations outside of the clinical environment You will work as part of a team making every contact count, taking every opportunity to impact directly and indirectly on the health and wellbeing of people. You will be able to work under direction seeking guidance where necessary and as an autonomous practitioner working within own limitations. Support the patient demand within the service, prioritising against the service specification and the patients clinical need in both step up and step down and in reaching into the hospital as required to support flow.
