The successful applicant will manage asmall, defined caseload and work closely with commissioning services and AdultSocial Care to provide the best outcome for our patients.

Main duties of the job
The PARTNER Repatriation Service provides thekey worker function/case management (previously care coordination) for patientswith complex mental health needs, and a high level of associated risks, who areplaced within out of county enhanced rehabilitation or specialist placements. This includes patients with a forensichistory.
The successful applicant will provideproactive support to our patients throughout their out of county admission andwill liaise with private service providers and stakeholders on a daily basis.You will work autonomously to ensure a clear recovery focus and effective careplanning is maintained, as well as supporting continued momentum andprogression through the care pathway for our patients. The successful applicantwill coordinate all aspects of complex discharges whilst working closely with ourcolleagues within Adult Social Care to support repatriation and discharge intoappropriate community based services.

Job responsibilities
* Coordinateclinical and support services relevant to discharge planning.
* Identify theneed for and coordinate MDT meetings/case conferences and support private careproviders in the decision making process regarding safe, effective and timelydischarge of patients.
* Carry acaseload and work collaboratively on the management of cases, ensuring liaison withcommunity services to support timely discharge from lockedrehabilitation/specialist placements.
* Participate inall aspects of discharge and aftercare including cooperation with Section 117.
* Ensure thatrisk assessments are carried out in conjunction with other agencies.
* Attendcommissioning panels acting as an advocate for patients.
* Establishand maintain communication with individuals and groups about complex dischargesovercoming any problems in communication.
* Involve,support, inform, and educate family/carers where appropriate throughout thedischarge process.
* Participatein/support the process of relevant assessments.
* Provideclinical supervision within the team where appropriate.

Person Specification
Experience
* Liaison with other agencies, e.g. social care, voluntary sector
* Experience of the funding processes for NHS funded care and social care
* Experience of conducting and contributing to ongoing assessment
* Experience of the CHC process.
* Experience of case management/care coordination
Qualifications
* RMN / Social Worker
* Degree / Diploma and working towards a first level degree
* Evidence of portfolio of knowledge, skills and professional development
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer
* LEO or other leadership courses.

Knowledge
* Knowledge of principles of assessment and treatment of individuals with complex mental health needs.
