Cheshire West & Chester Council are seeking an experienced and motivated social worker to join their Adults with Learning Disabilities Team. This role offers an exciting opportunity to make a real difference in the lives of adults with learning disabilities by providing tailored support and ensuring they can live as independently as possible.
Based in the Chester and Ellesmere Port area, you will be part of a dynamic team committed to delivering excellent social work practice while benefiting from hybrid working arrangements and supportive colleagues.
Responsibilities
1. Undertake comprehensive assessments to identify the needs and strengths of adults with learning disabilities.
2. Develop and implement person-centered care and support plans to promote independence and well-being.
3. Work collaboratively with individuals, their families, and multi-agency partners to deliver effective interventions.
4. Support the team's duty rota, responding promptly to urgent cases and ensuring continuity of care.
5. Apply relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding policies, to uphold the rights and safety of service users.
6. Maintain accurate and timely records in line with council policies and procedures.
Requirements
1. Qualifications: Degree in Social Work and registration with Social Work England.
2. Experience: Preferably with experience working in learning disabilities, including assessments, care planning, and safeguarding.
3. Skills: Strong communication, organisational, and analytical skills, with the ability to manage a varied caseload effectively.
4. Knowledge: Sound understanding of relevant legislation, policies, and best practices in adult social care.
5. Flexibility: Ability to work in a hybrid model, with at least two days per week in the office.
