Job summary
Following the release of ARRS GP information and funding weare looking for a newly qualified (within 2 years and not having had asubstantive post), enthusiastic, forward-thinking GPs to join Bradford City PrimaryCare Network 4 on a fixed term salaried contract until 31 March 2025 with thepossibly of extended further dependant on ARRs Funding. We will considerapplications for between 4 and 8 sessions with a start date as soon aspossible.
You will be required to work across 2-4 practices (dependingon the number of sessions contracted for) supporting our patients through thewinter period. All of our practices are rated good with CQC, some of which aretraining practices. We are a highly supportive PCN, and the practices all workclosely together. We understand the importance of a good work/life balance withan open-door ethos.
Main duties of the job
To undertake duties governed by the PMS &GMS contract and Objectives of Direct and Local Enhanced Services including:
Face to face and telephone consultations.
Patient Home visits and Nursing Home visits.
Checking and signing repeat prescriptions.
Administration - dealing with queries, patienttest results, referral letters NHS/private, task, emails and other paperworkand correspondence in a timely manner.
Cover all the clinical management of thepatients in surgery as appropriate including initiating investigations,reviewing results, making referrals to secondary care or to other providers asappropriate.
Develop care and treatment plans in consultationwith patients and in line with current Practice disease management protocols aswell wider guidance such as that from NICE2-3.

Recording clear and contemporaneous based consultationnotes to agreed standards.
Provide safe, evidence-based, cost-effective, individualisedpatient care within the surgery, patients own home or other environment wherepatient care is carried out.
Clinically exam and assess patient needs from aphysiological and psychological perspective, and plan clinical care accordinglyreferring patients to other services/agencies in a timely manner, utilisingPractice, PCN and local guidelines.
To recognise the need for and be able to perform andinterpret investigatory procedures, including laboratory and radiology.
Prioritise health problems and intervene appropriately toassist the patient in complex, urgent or emergency situations, includinginitiation of effective emergency care.
Ensure appropriate follow up of patients.
Support patients to adopt health promotion strategies that promotehealthy lifestyles and apply principles of self-care.
Support the delivery of anticipatory care plans.
Recognise and work within your own competence and inaccordance with professional codes of conduct, maintaining accurate andcontemporaneous health records.
Independently prescribe for patients within your scope ofpractice. Prescribing in accordance with locally agreed or national guidelines.
Compiling and issuing computer-generated acute and repeatprescriptions.
Review medications following the appropriate policies,NICE/local clinical guidelines and local care pathways.
To maintain an awareness of developments in clinicalpractice.
Awareness of and compliance with all relevant practicepolicies/guidelines prescribing, confidentiality, data protection, healthand safety and QOF standards.
Be aware of your duties and responsibilities regardingcurrent legislation and adhere to our policies and procedures on SafeguardingChildren and Safeguarding Adults.
