Wesham Rehabilitation centre is advertising an exciting post within Mental Health Rehabilitation clinical pathways, with this will be a great opportunity for individuals to work as part of this team and initiative.
Wesham rehabilitation centre provides a model of rehabilitation care for 24 males which focuses on empowering the service user to gain or recover their confidence and skills that promote independence, autonomy and encourage hope for the future, whilst in recovery of a mental health illness. Encouraging the skills necessary to transition back into a community setting.
We provide a welcoming and domestic feel to service users. The facility is registered with the CQC as Open Community Rehabilitation service, registered to accommodate service users detained under the MHA. There is access to full MDT including nursing staff, occupational therapists, psychology, medical staff, physicians associate.
Applicants must have experience and an expressed interest in working within the rehabilitation pathway.
You will be a Nurse working within a multi-disciplinary team and carry out a diverse range of duties. As a Band 5 Nurse within the team you will be regularly supported and supervised by a nominated Band 6 Nurse and you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
You will actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct. You are required to maintain links with your professional body and provide evidence for the revalidation process.
You will work within the multi-disciplinary team to ensure the effective assessment, formulation, planning and monitoring of recovery focused rehabilitation care directed to service users.
Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
