Community Support Worker - Central London Community Healthcare NHS Trust
An exciting opportunity has arisen for a Band 3 Community Support Worker (CSW) to join Barnet Unplanned Care Service. We have several pathways within the service such as D2A, NWB, Delirium, Rapid Response and Enhanced Nursing as well as the Hospital at Home.
The Unplanned care team provides short term support to help patients manage the short term transition from hospital to home and regain their independence.
The Community Support Worker role will focus on supporting therapists/Nurses in the Team to improve patients' independence. The role will include promotion of daily living skills, simple Home Exercises Programme (HEP) to improve balance/muscle strength and personal/social care support.
The successful post holder will perform nursing and therapy procedures where necessary for which adequate training has been given. They will work under the supervision of the professional staff in the Unplanned care team to agreed standards of care considering the needs and wishes of the patient and their families.
To be highly organised with excellent verbal and written communication skills and experience of being able to follow Therapy and Nursing care plans and treatment plans in a community environment. Successful post holder must have an ability to work 37.5 hours across 7 days per week. Hours of work are 8am - 8pm on two shift patterns: 8am - 4pm and 12pm - 8pm.
Responsibilities include:
1. Carrying out pre-defined care plans with patients in their homes, as directed by a senior support worker or qualified clinician for a range of patients promoting an enablement ethos where appropriate, ensuring that patient safety and care is always prioritised.
2. Reporting back to the allocated senior support worker, clinician or therapist, and documenting effectively care tasks undertaken, progress made, outcomes of intervention and any concerns regarding patients' health, wellbeing, or ability.
3. Utilising basic health observation techniques to monitor patients' health status as appropriate and directed by senior staff, reporting back findings.
4. Supporting patients with ADL, personal care and meal preparation services and assisting patients who require support with these tasks.
5. Ensuring all aspects of work are carried out to a high clinical standard and being accountable for this through the senior staff.
