Main duties of the job
Provide high quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way whilst maintaining the patient's privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the Community Nursing team in the management of the health care environment.
Implement/refer to the relevant services as appropriate.
Work closely with the Integrated Community Teams/GP surgeries and local acute Trusts.

Education, Qualifications
* Nurse Associate qualification / working towards
* NMC registration / working towards
* Community experience
* Wound Care experience
* Lower Limb training
Experience and Knowledge
* Working in health and or social care environment
* Understanding of factors that influence health and wellbeing
* Following treatment plans
* Risk management and safeguarding
* MDT or integrated team working
* SystmOne experience or other electronic patient documentation system
Skills and Attributes
* Effective time management
* Communication
* Understanding of lower limb compression bandaging
Aptitude and Personal Qualitie
* Team player but able to work alone.
