Job Summary
The Specialist Clinical Pharmacist -- Medicines Optimisation will play a key role in supporting the Medicines Optimisation team in delivering medicines management and optimisation across the Trust.
The post holder will assist in horizon scanning, with specific interest in the implementation of cost-effective therapies as product patents expire commercially. The post holder will:
1. Lead on coordinating, monitoring and analysing medicines related cost improvement programmes.
2. Utilise prescribing skills to expedite switch programmes.
3. Support the relevant clinical MDT and be actively involved in the service to patients being switched to cost-effective treatments within specialties across the Trust.

Main Duties of the Job
The successful post holder's main responsibilities will include:
1. Ensure correct and safe prescribing, demonstrate expert clinical knowledge and provide a clinical pharmacy service of the highest standard.
2. Proactively facilitate switching patients' biologics to biosimilars.
3. Provide a clinic-based pharmacy service to patients, carers, relatives alongside other health care professionals involved in the treatment of patients.
4. Undertake a pharmacist-led drug education clinic.
5. Provide advice and information on issues surrounding adherence to medications prescribed in the treatment of patients.
6. Develop protocols and guidelines to support the medicines optimisation process where appropriate.
7. Help develop new methods of delivering patient-focused care with the multidisciplinary team.
8. Provide risk management strategies and ensure compliance with medicines legislation and clinical governance.
Additionally, the post holder will support monitoring drug use and expenditure, ensuring this information is disseminated to the relevant clinicians, pharmacy and finance personnel. The role will also include the development and collation of the dataset (in collaboration with informatics and finance) required for submission to the Trust as evidence for compliance.
