We are looking for passionate, enthusiastic & motivated Registered Nurses to join our innovative Neighbourhood Nursing Service throughout the Barnsley area.
The service is responsible for delivering a wide range of nursing care to housebound patients, which includes wound management, chronic disease management, promotion of self-care, terminal/palliative care & the administration of IV anti-biotics for admission avoidance.
Successful candidates will maintain high quality effective nursing care in the community as part of the wider integrated care team.

Main duties of the job
As a Neighbourhood Staff Nurse you will support the District Nurse in carrying out nursing care for patients primarily within their home but you will also participate in clinic sessions.
Enthusiasm and the ability to use your own initiative are essential, along with good written and verbal communication skills and the ability to manage your time effectively.
You will need the ability to work both as part of a team and as a lone worker, along with the use of a vehicle to undertake any necessary travel in connection with the duties of the post.
You will be allocated to a specific Neighbourhood; however, it should be noted that you are employed to work across the service and may be required to move bases if/when required.
You will be required to work flexibly as the service operates 8am - 8pm.

Job responsibilities
1. Undertake duties under the supervision of the District Nurse.
2. Deliver evidence-based planned care, which is safe, effective, in a manner which is caring, compassionate and enables individuals to reach an optimum level of functioning, independence, rehabilitation and recovery.
3. Adapt and provide a wide range of nursing care in home and community-based settings.
4. Be accountable for assessing, planning, implementing and evaluating care for patients, reporting and escalating relevant information to appropriate health and social care professionals.
5. Work in an integrated and partnership way with primary, secondary, social care, the independent and voluntary sector and others to improve the health and care of individuals, families and communities, particularly the most vulnerable.
6. Undertake and maintain the required statutory, mandatory and core training and preparation to meet the specific clinical needs of the service.
7. Participate with the training of learners and participate in staff inductions where appropriate.
8. Delegate work to other staff members within the team.
9. Establish good working relationships with the Primary Health Care Teams.
10. Act as the named nurse under the supervision of the District Nurse, including the first and continuing assessment, planning, implementing and evaluation of care given to individuals in their own homes.
11. In the absence of the District Nurse, manage the caseload and the District Nurse Team, monitoring standards of care and deploying and supervising team members as appropriate.
12. Participate in the planning and delivery of packages of care, including liaison with other health professionals and statutory and voluntary agencies to deliver skilled nursing care and support any changes.
13. Record all patient contacts and activities accurately and contemporaneously maintaining accurate records and statistical returns as required by the Trust including entering data onto the Trust's systems within the required timeframes.
14. Advise patients on health promotion issues in order to achieve pre-determined local and national health targets.
15. Participate in health screening programmes and disease management of patients under the direction of the District Nurse.
16. To be a skilled communicator establishing and maintaining excellent relationships with members of the Primary Health Care Team and other Health and Social Care professionals, initiating referrals where appropriate to ensure adequate support for patients and carers.
