Job Purpose Virtual wards support patients, who would otherwise be in hospital, to receive the acute care, remote monitoring, and treatment they need in their own home or usual place of residence. This will consist of a mixed caseload of the step down from hospital whereby patients are still under the care of a hospital consultant and step up from the community where the medical oversight is provided by NEMS GPs. Virtual wards provide acute clinical care at home for a short duration (up to 14 days) as an alternative to care in hospital. The overriding purpose of the Virtual Ward Matron is to support the provision of the highest quality patient care to individuals whilst on the virtual ward pathway, supporting the service manager. The post holder will support the virtual ward registered staff and clinical team ensuring the delivery of high-quality patient care upholding patient dignity delivered with kindness and compassion. Clearly demonstrating a clinical leadership role, with responsibility for planning, co-ordinating and delivering care as appropriate per the daily MDT, initiate and manage medical and nursing care interventions, co-ordinating the care of people with highly complex needs lead care and co-ordinate partner agencies and stakeholders. The successful candidate will hold the NMP qualification or be ready to undertake this within 12 months of being in post. The Virtual Ward Team core hours are 08.00 20.00 over 7 days, covered by a shift pattern of 08.00-16.00, 10.00 18.00 and 12.00 20.00 also working an average of 1 weekend in 4. Each clinician will be expected to be part of the Rota. Role Context: To provide appropriate and effective care management through the virtual ward pathway unnecessary hospital admission from the Community managing the active caseload and providing necessary oversight and support Demonstrate/develop advanced skills and competencies in prescribing, health assessment, medicines management, leadership and management. To holistically assess the needs of patients and carers, plan, implement and evaluate nursing care interventions. Embrace multi professional and partnership working to facilitate co-ordination, promotion of optimum health and social outcomes independence and self-care. Undertake virtual monitoring and escalate concerns to the daily MDT.
