We are looking for a strong, dynamic and enthusiastic leader, with excellent knowledge of Preventative practice and experience of working in Adult Social Care. Experience of supporting change management would be desirable.
The Senior Practitioner will be able to use their role to support a cultural change within existing staff through coaching, mentoring, direct supervision developing the skills of existing staff across a number of partners to improve the interface and support individuals to achieve their wellbeing outcomes
This should result in an increase in people meeting their own needs, signposted to relevant services or supported to access integrated preventative services that reduce hospital admissions, long term care admissions or increased packages of care.
