Primary Duties and Key Responsibilities:
1. Ensure that all clinical assessments are completed on first assessment and reviewed regularly, especially when the patient's condition changes.
2. Identify when individual risk assessments are required and liaise with an appropriately trained member of staff to ensure that these are completed.
3. Involve patients in all aspects of their care as much as they are able and act as a patient advocate when required, ensuring that those important to the patient are involved in their care in accordance with the patient's wishes.
4. Provide symptomatic relief if required by administering prescribed medications.
5. Develop and maintain effective communications with patients and those important to them, in a way that respects their views, autonomy, and culture, and contribute to emotional support during admission, at the time of death, and bereavement.
6. Ensure that a suitable and safe environment is provided, whether on the ward or in the home environment.
7. Lead shifts on the IPU when required, ensuring appropriate allocation of workload for the IPU and Community team when necessary.
8. Supervise the clinical practice of Healthcare Assistants and participate in the orientation of new staff, including Bank or Agency staff.
9. Act as a mentor for work placements and visiting trainees.
10. Maintain nursing records according to mandatory legal requirements, ensuring confidentiality is maintained at all times in accordance with NMC Guidelines Standards for records and record keeping.
11. Complete pharmacy requisitions as per policy and Standard Operating Procedures.
12. Liaise with other healthcare partners such as Community Nursing teams, Continuing Healthcare, GPs, Community Physiotherapists, and Occupational Therapists.
13. Prioritise workload according to changing clinical needs for the shift.
14. Contribute to setting goals and benchmarking standards of care.
15. Develop tools to improve the quality of the service and coordinate clinical services across the ward.
16. Record and report all incidents/complaints involving staff, patients, and visitors in accordance with Trust policies and assist in any investigations as required.
17. Work to ensure that clinical areas are well maintained and comply with health and safety regulations, ensuring the safe use of equipment and storage of materials.
18. Comply with the NMC Professional Code of Practice and Behaviour for Nurses and Midwives.
