Job summary
The post holder will be an experienced Advanced Nurse Practitioner who will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care.
The successful candidate will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures whilst demonstrating safe, clinical decision making. In order to work at this level, NMC requirements for advanced practice must be met.
Main duties of the job
1. To deliver a high standard of patient care as an Advanced Nurse Practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base
2. To manage a clinical caseload, dealing with presenting patients needs in a primary care setting
3. To develop the nursing services offered by the practice
4. Prescribe safe, effective and appropriate medication as defined by current legislative framework
5. To manage own clinical workload in general practice responding effectively to patient need and ensuring patient choice and ease of access to services.

The post-holder will :
6. Provide the choice of direct access to an Advanced Nurse Practitioner, both in the Practice and over the telephone, for the general practice population
7. Make professionally autonomous decisions for which he/she is accountable
8. Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan
9. To maintain accurate and contemporaneous records, utilising computer systems where appropriate and consider the Caldicott Principles in relation to all data handling.
10. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
11. Provide safe, evidence-based, cost-effective, individualised patient care
12. Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care
13. Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills
14. Refer patients directly to other services/agencies as appropriate
15. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery

16. Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
17. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review
18. Maintain their professional registration
19. Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained
20. Work within the latest NMC Code of Professional Conduct
21. Record accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards
22. Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation
23. Work collaboratively with colleagues within and external to the practice.
24. Pro-actively promote the role of the Nurse Practitioner within the Practice, and externally to key stakeholders and agencies
TEAM ROLE
25. As a clinician be available to support other healthcare team members and to accept referrals from other team members.
26. To liaise with all members of the Primary Health Care Team and other agencies local authority, social services, secondary care, voluntary sector and Primary Care Trust in order to assure appropriate care is provided for the practice population.
27. To participate as a key member of the multi-professional team through the development of collaborative and innovative practice.

29. To delegate appropriate tasks to the primary care team whilst still maintaining responsibility for this area.
30. To support the delivery of health and safety guidance as agreed within the practice.
ORGANISATIONAL
To complete records, audits, reports and respond to appropriatequestions and requests.
To attend meetings as requested.
Support effective communication channels between the whole team.
To work according to Clinical Governance and support the ClinicalGovernance Agenda.
To understand the role of the Risk Management Team within the practicecontributing to risk assessment and Significant Event Audit and the importanceof Infection Control.
The individual may be required to undertake the assessment andmanagement of patients within the home environment.
You will be expected to deal appropriately with all admin work thatarise from your caseload, including actioning lablinks, tasks and documentsfrom hospital and other professionals, interpretation and actioning of othertest results.
