In conjunction with the haemoglobinopathies team across King's College NHS Foundation Trust, the post holder will be responsible for the ongoing development and delivery of a high-quality haemoglobinopathies service across the Trust.
King's College NHS Foundation Trust has been designated as a Haemoglobinopathy Coordinating Centre for sickle cell disease, thalassaemia, and other rare inherited anaemias. The postholder will be an experienced nurse leader and responsible for triaging sickle cell patients presenting to King's College NHS Foundation Trust, coordinating the most appropriate location for their care across inpatient, emergency, and day care settings.
They will provide clinical leadership and practice at an advanced level, demonstrating a high level of autonomy and clinical decision-making. They will be expected to provide excellent customer care for patients, which may include communication with distressed and anxious patients and relatives, treating them with tact and empathy, ensuring the professional image of the Trust is maintained at all times. You should be flexible, enthusiastic, passionate, and an excellent communicator. This role is also ideal for anyone who wishes to make a difference to patients' experience while supporting and collaborating with the wider team structure to improve patient experience.
Main duties of the job
1. Provide timely and effective assessment and triaging of patients presenting with sickle cell crisis, supporting and caring for patients.
2. Provide timely and effective assessment and triaging of patients with Thalassaemia.
3. Support the care of adolescents transferring to and accessing the adult Haemoglobinopathies service.
4. Provide expert clinical advice, support, and guidance for patients, carers, and allied healthcare professionals involved in providing care for patients with sickle cell disease and Thalassaemia.
5. Offer timely and appropriate clinical decision-making and interventions where appropriate and escalate as required.
6. Work closely with haematologists, bed managers, ward managers, and nurses to initiate timely admission and discharge planning.
7. Facilitate nurse-led discharges.
8. Act as a role model and clinical resource, demonstrating high standards of care and providing clinical leadership in the specialist area.
9. To liaise with relevant A&E staff and other members of the MDT to optimize a smooth treatment pathway for patients in ambulatory care triage.
10. Develop and run nurse-led clinics, e.g., Hydroxycarbamide/iron chelation.
11. Support Apheresis in performing emergency procedures as clinically required.
