Advanced Nurse Practitioner / Advanced Clinical Practitioner
Northgate Surgery is looking for an Advanced Nurse Practitioner/Advanced Clinical Practitioner to join our team. We are looking for 20-37.5 hours per week. Highly competitive salary. Must be able to work Mondays.
We are a friendly, advanced training practice with 4 partners, 7 salaried GPs, GP trainees, 5 Advanced Nurse/clinical practitioners, medical students, pharmacists, and an excellent nursing and administrative team.
Main Duties of the Job
1. Work within our current ACP team dealing with acute and routine patient problems which will consist of face-to-face, telephone, online consultations, and home visit appointments.
2. Efficiently manage the recalls and reviews of patients with chronic diseases.
3. Receive patients in clinic, including assessment, diagnosis, treatment, prescribing, and referral as appropriate. Provide public health, health protection, and promotion programmes that improve health and reduce inequalities.
4. Conduct home visits to patients who are appropriate to be seen by an Advanced Clinical/Nurse Practitioner in their own homes or in a care home setting. Carry out health reviews, medication reviews, and care planning.
5. Ensure that significant events are reported using Practice procedures.
About Us
We have a fantastic opportunity for an enthusiastic Advanced Nurse/Clinical Practitioner to join our team. Northgate Surgery is a friendly and supportive working environment. The Practice has a patient population of 15,400 and is still growing. The Practice is based in the centre of Pontefract and provides parking.
We are a training practice and play an active role in our local PCN, with excellent clinical, admin, and management support teams to support the right individual in the role.
Job Responsibilities
1. Work with the GPs/Practice Manager/ACP team to run the appointment system, advising on the best methods to obtain the maximum benefit to our patients.
2. Telephone triage patients with undifferentiated and undiagnosed problems and assess their healthcare needs. Book patients into your own clinics.
3. Maintain accurate patient records and enter onto the computer using agreed Read Codes and ensure all nursing staff do the same.
4. Collect data and participate in regular Practice audit and research projects.
5. Contribute to the preparation of any Practice development plans.
6. Maintain own 3 yearly registration with the NMC and adhere to the NMC Code of Professional Conduct.
7. Maintain a safe environment for all Primary Health Care staff, patients, carers, and co-workers from other agencies, ensuring adherence to all statutory regulations (e.g. Health & Safety, COSHH, etc).
8. Represent the ACP team in a professional manner by attire and attitude to patients and staff both in and out of the Practice.
9. Maintain own continuous professional development and ensure PDP is kept up-to-date in line with Practice requirements.
10. Attend and participate in staff meetings.
11. Any other reasonable duties that may be required from time to time.
