Specialist Speech and Language Therapist

We are excited to be advertising for a 12 month Fixed Term 0.6 WTE Band 7 Specialist Speech and Language Therapist with a specialist interest in young people with SEMH. This role will be part of the Managing Mood, Emotions and Relationship Pathway, working closely with colleagues in mental health but with strong professional links to speech and language therapists across CFHD. This role can be based within any of the three CFHD localities (North Devon, Exeter, East and Mid Devon & Torbay and South Devon).

The post holder will take a key role in the language assessment and support of children and young people and their families/carers within the MERS pathway across the recruiting locality. The successful applicant will have experience and training relevant to this role and be able to support others in developing their own competencies.
Main duties of the job
* Work within a CFHD pathway of care providing support to speech and language therapists across the designated pathway/s of care.
* Screen and triage referrals and ensure a strengths-based approach to assess, plan, implement and evaluate interventions to support the complex packages of care for children with additional needs in the community.
* Contribute to risk assessment and care planning in collaboration with children, young people and their carers.
* Provide specialist SLT assessment, advice and interventions to children, young people and their families and to hold a caseload of children with complex needs.
* Provide case management of an identified caseload of children and young people with highly complex long-term conditions and complex needs by utilising and promoting evidence-based care. This includes children with complex needs including speech, language and communication.

Children and Family Health Devon (CFHD) comprises an Alliance of NHS organisations working together to improve health outcomes for children and young people in Devon. CFHD provides integrated care and treatment for children and young people across physical and mental health which are provided by Torbay and South Devon Foundation Trust and Devon Partnership Trust. We are working closely with other organisations and providers to ensure we deliver locality and county-wide based integrated access to all of our services, based on the i-Thrive Framework, in response to consultation with our service users and communities.
CFHD works closely with our local universities of Plymouth, Exeter & the Peninsula Medical School where we support students in clinical training placements and in addition support our own staff to undertake post graduate training. Our integrated care and treatment has been developed and refined in partnership with children, young people and their families/carers.

Job responsibilities
* Act as a source of expertise on their specialist clinical area which may include speech, language, communication and/or eating, drinking and swallowing difficulties.
* Advocate on behalf of CYP and to be at the forefront of clinical development within their specialist clinical area.
* Effectively communicate with children, families and staff to be able to sensitively impart difficult news and respond appropriately to complaints and challenges.
* Demonstrate excellent interpersonal and communication skills with the whole of CFHD.
* Represent speech and language therapy and/or individual patients to ensure the delivery of a coordinated multi-disciplinary service and to integrate Speech and Language therapy management into the overall management plan. This may involve health/social care staff, voluntary and private sector, and will include providing specialist advice to the multi-disciplinary team on management of recovery and discussion of patient care, progress and discharge planning.
* Communicate complex and sensitive condition-related information from assessment to children/young people and families, carers and members of the multidisciplinary team/other professions in a way that is accessible and easily understood. This will include facilitating strategies for people with communication disorders where barriers to understanding may exist.
* Demonstrate empathy with children/young people, families, carers and colleagues, ensuring that effective communication is achieved, particularly where barriers to communication exist, or sensitive issues are being dealt with.

ESSENTIAL
* Bsc or Diploma in SALT
* Registration (HCPC)
* Evidence of training/CPD maintained in relation to role (SM)
* Experience of working in a specialist field
* Post graduate Leadership
* Membership of Professional Body
* Evidence of quality improvement
