Job overview
The Children’s Rapid Response Service has an exciting opportunity as it continues to expand the team. The Children’s Rapid Response Service works collaboratively with the Community Children’s Nursing & Palliative Care Team. The primary aim is to prevent and reduce hospital attendances, admissions, and length of stay for children already known to the Community Children’s Nursing & Palliative Team.
The emphasis of this service is to improve the quality of life for children and families with complex healthcare needs, offering them greater choice around their chosen place of care.
Are you an enthusiastic, dynamic, flexible, and motivated experienced Nurse or Nurse Practitioner with a strong knowledge of Children’s Nursing? Clinical expertise and good communication skills are required, as well as excellent time management and prioritisation skills which are essential for this role. The service is open from 07:30 – 22:00 across 7 days and there is flexibility to work short shifts or long days.
Main duties of the job
We are looking for experienced children’s nurses with extensive experience in assessment with children with complex healthcare needs.
You must have the ability to prioritise your workload using advanced assessment skills, providing a symptom management plan of care to children/young people during times of acute deterioration or illness.
The role requires undertaking clinical assessments of a sick child or young person and emergency enteral feeding management to prevent hospital admissions.

Detailed job description and main responsibilities
Professional
* Work in accordance with NMC Code of Professional Practice and Scope of Professional Practice.
* Work within the community (e.g., within client’s own home) as a lone practitioner managing situations of risk with telephone support.
* Provide a calm and supportive approach to the management and delivery of sensitive and complex information.
* Use effective communication techniques to ensure the child and family referred to Children’s Rapid Response receive the best possible care.
* Undertake risk assessments prior to service delivery within the home setting to ensure staff safety.
* Work in partnership with children, families, and carers to provide optimum individual outcomes.
* Formulate individualised management and treatment plans using sound knowledge and evidence-based practice.
* Provide timely, effective symptom management, resulting from comprehensive history taking.
* Promote strategies/detailed plans of care to parents and carers.
* Ensure assessment, planning, and delivery of holistic care is of the highest standard.
* Liaise and work in collaboration with acute services, primary healthcare team, and other agencies.
* Advise the Team Leader, Clinical Lead, or Service Manager of any concerns in relation to patient care.
* Maintain clear, concise, and contemporaneous records adhering to BCHC record-keeping policies.
* Provide a flexible service to accommodate the changing needs of children and families.
Management
* Provide leadership in the absence of the Team Leader/Clinical Lead.
* Responsible for accepting referrals and discharging patients from own caseload.
* Ensure standards are maintained with clinical quality and governance.

* Participate in research and audit relevant to the service.
* Manage the service resources effectively.
* Develop policies, procedures, and guidelines.

Person specification
Qualifications / training
* RSCN/RN Child Branch
* ENB 998 (Teaching & Assessing in Clinical Practice) or equivalent
* Community Children’s Nursing qualification or equivalent
* Clinical assessment course equivalent
* Independent supplementary prescriber or equivalent
Experience
* Experience of working with children with complex healthcare needs.
* Significant experience as a Children’s Nurse either RSCN/RN Child.
Skills/knowledge
* Knowledge of child protection issues and procedures.
* Evidence of assessment skills and use of nursing process.
* Sound use and application of NMC Code of Conduct.
* Knowledge of current legislative changes affecting NHS and Social Services.
* Knowledge and application of current clinical trends and practices.
* Ability to teach relatives, patients, clients, and staff.
* Demonstrate individual responsibility in respect to health and safety.
* Ability to demonstrate written skills that are concise, accurate, relevant, and legible.
