We are looking to appoint nurses as Clinical Commissioning Case Managers working across the Birmingham and Solihull area; supporting individuals who are eligible for NHS Continuing Healthcare. The role requires excellent time management, effective communication skills, and the ability to work collaboratively with a range of health and care professionals to ensure the care needs of individuals with a wide range of conditions are being addressed in a safe, appropriate, and effective manner.
We are a clinically led service, and we will offer support to develop your skills and knowledge in this very skilled area of health provision. This role offers an opportunity to support some of the most vulnerable and complex individuals who live in our community, improving their outcomes and quality of life.
Main duties of the job
1. The post holder will be required to provide expert advice and assist in identifying appropriate providers to address the needs of complex individuals.
2. The post holder will be required to attend Multi-Disciplinary Meetings, Acute Care Facilities, individuals' homes, and a variety of care provider settings.
3. The role will be required to regularly review individuals in line with The National Framework for NHS Continuing Healthcare, case management requirements.
4. To present comprehensive written reports providing assurance to the ICB that the agreed placement is delivering the care to meet identified needs and it is appropriate to continue commissioning the care plan.
5. They will be responsible for negotiating care costs and following the agreed ICB processes for the agreement of care packages.
6. To support the Head of CHC and Lead Nurses in ensuring appropriate individual placements provide safe and high-quality care.
7. Responsible for the line management of a defined number (up to 4) of Band 6 Clinical Nurse Assessors.

Job responsibilities
1. The post holder will be required to provide a high-quality resource of professional advice, guidance, and expertise to members of the AACHC, ICB, and wider health and social care economy.
2. Autonomously manage a caseload of individuals with highly complex health needs, providing significant clinical guidance to providers.
3. Undertake regular reviews supporting commissioned care, PHB, and ongoing case management.
4. Analyze supporting documentation to establish the critical history of the patients' relevant needs to produce a specialist nursing assessment.
5. Have an expert level of knowledge regarding The National Framework and its application in practice, and be able to impart this knowledge to a variety of audiences.
6. Provide complex and sensitive information to patients, relatives/representatives, and often work in potentially hostile environments; relaying difficult decisions regarding care provision and managing patient and family expectations.
7. Attend all meetings related to the patient which may impact on the care provision and costs to meet identified needs.
8. Liaise with multiple professionals involved to ensure that a comprehensive understanding of all identified needs is available.
9. Ensure that each patient has a care plan that addresses all their identified needs and can be used to assist assessors when completing CHC assessments/reviews.
10. Be responsible for negotiating care package costs and monitoring packages of fully funded care.
11. Ensure these care packages are reviewed to an agreed time scale, identified by the senior clinicians, and that these events are clearly recorded on the ICB database.
12. Identify and process any safeguarding and quality issues and take appropriate action to ensure that people's welfare is protected.
13. Ensure the Mental Capacity Act is considered and ensure relevant processes are followed, e.g., Best Interests, DOLs.
14. Provide a high-quality resource of professional advice, guidance, and expertise to the ICB and partner agencies in addressing the needs of individuals with complex health needs.
15. Call and chair multi-disciplinary meetings, including case conferences and discharge meetings to ensure continuity of care across agencies; thus improving patient and family experience.
16. Deputise for the Clinical Services Delivery Manager as required.
17. Contribute to and deliver training on CHC and FNC to a variety of audiences, particularly in health and social care.

Qualifications
Essential
* Educated to degree level or equivalent level of experience working in a specialist area.
* Professional Qualification and current registration with relevant professional body i.e., NMC.
* Registered Nurse.

* Previous experience of undertaking all-age Continuing Care Assessments.
* Experience of negotiating and liaising with a multi-disciplinary team.
* Experience of Complex Case Management.
Knowledge
Essential
* Knowledge of MCA, DOLS, LPS.
* Good understanding of Safeguarding issues and policy framework.
* Good understanding and/or experience of CHC National Framework.
Desirable
* Experience of commissioning.
* Experience of line management of staff.
