Job Opportunity: Adults Social Worker – Mental Health Service Location: Wrexham, North Wales Salary: £43 per hour Contract: Temporary, Full-time Closing Date: Ongoing About the Role Wrexham Council is seeking an experienced and compassionate Adults Social Worker to join our Mental Health Service team. You will work as part of an integrated team, providing high-quality social work support to individuals with mental health needs, ensuring they receive appropriate interventions, services, and support to improve their wellbeing and independence. As an Adults Social Worker within our Mental Health Service, you will work collaboratively with multidisciplinary teams, including health professionals, to provide tailored support to adults with mental health needs, ensuring that their care plans align with both their needs and preferences. Key Responsibilities: Conduct thorough assessments of adults with mental health conditions, identifying their strengths, needs, and risks in accordance with relevant frameworks and legislation. Develop, implement, and review care plans that reflect individual needs, ensuring that service users are involved in the planning and decision-making process. Work in close collaboration with mental health professionals, including psychiatrists, psychologists, and community health teams, to deliver integrated and person-centred care. Ensure that safeguarding procedures are followed, working to protect vulnerable adults from harm and ensuring their rights are upheld. Provide support for individuals transitioning between services, ensuring a smooth process and continuity of care. Offer guidance and support to service users and their families, promoting independence and self-management. Maintain clear and accurate case records in line with statutory, regulatory, and Wrexham Council policies. Participate in care reviews, multi-disciplinary meetings, and planning meetings to ensure the most effective services are provided. Advocate for service users, ensuring their voices are heard and that they receive the necessary support and care. What We Are Looking For: HCPC-registered Social Worker with current Social Work England registration. Experience working within adult social care, particularly in mental health services, and a strong understanding of relevant legislation such as the Care Act 2014 and the Mental Health Act 1983. Knowledge of mental health conditions and the challenges faced by individuals with mental health needs. Ability to assess complex situations and provide practical, evidence-based solutions. Excellent communication skills, with the ability to build trusting relationships with service users, families, and multi-disciplinary teams. Experience in safeguarding vulnerable adults and implementing risk management strategies. Strong organisational skills and the ability to manage a caseload effectively.
