Job summary The Integrated Hospital Discharge Services purpose is to support people transferring from hospital to recover and maintain their independence and wherever possible, to return and stay in their own homes. You will lead and manage a multi-disciplinary staff group of managers and social care professionals, to achieve positive outcomes and fulfilled lives for people leaving hospital and whilst recovering post discharge. You will work with teams to enable people to achieve these outcomes working across health and social care organisations seven days a week.
