We are looking for a highly motivated and enthusiastic Senior Practitioner interested in the challenges of working in our Hospital Discharge Team and who can bring their knowledge, skills and experience. The team is based within East Lothian Community Hospital and works closely with NHS colleagues. The role of the team is to assess adults in hospital who may require to move into a care home or support on discharge home and also provides a review and adult protection service to adults living in care homes. The Senior Practitioner acts as a line manager within the team, overseeing the work of Social Workers and CCWs and providing supervision and support.
