The bedded centre focusses on providing high quality, integrated care for patients from within the local adult population with sub-acute health care needs.
Main duties of the job
You will provide clinical and professional leadership for a ward-based team of registered nurses and healthcare assistants and deputise for the Ward Manager in their absence.
The person we are looking for will be:
1. a registered nurse with current professional registration

3. an excellent communicator who will innovate, inspire and promote patient safety whilst supporting the ward team.
4. an advocate for our patients, able to identify changes in need and be instrumental in planning for their current and future care.

About the Community Intermediate Care Centre (CICC)
The Centre is an integrated team of clinical staff and professionals including Physiotherapists, Occupational Therapists and Social Workers. CICC supports the recovery and recuperation of our patients following medical and surgical illness and also provides palliative care and manages the planning of complex discharge packages. One of the fundamental principles of the centre is to support our patients with their reablement and self-care, encouraging them to reach their optimum level of independence.

Job description
Job responsibilities
You will support the Team Leader with efficient management and day to day running of the ward and monitor the assessment, development, implementation and evaluation of programmes of care for patients. in co-operation with the multidisciplinary will be able to demonstrate your professional skills and competence in leadership, care planning / co-ordination, medicines management, and care and treatment will support the Team Leader with delivering a high quality efficient and effective service within the ward, meeting defined performance indicators including quality standards, quality goals and patient will be involved in both management and the individual professional development of the clinical team incorporating the Trust's appraisal will provide advice and support to junior staff in the delivery of high quality care and actively support the Ward Manager inthe management of the team.

Qualifications and Professional Training
Essential
18. Registered General Nurse educated to degree level (or equivalent)
19. Specialist Practitioner Qualification - District Nursing (or equivalent postgraduate modules) or equivalent level nursing qualification/ experience
20. Current NMC registration
21. Evidence of further recent study/professional development
Desirable
22. Independent non-medical prescriber or willing to undertake
23. Teaching/Mentorship Qualification
24. Clinical examination module (or willing to undertake)
25. Leadership Qualification - ILM level 6 or equivalent (or willing to work towards) or relevant experience
