Job Description:

This is a fulltime, permanent post of 37 hours per week, with a shift pattern Monday to Friday.
The Community Mental Health Team is looking to recruit Band 5 Community Mental Health Nurse. The postholder will be an integral part of the Multi-disciplinary Team, who will work in partnership with all service users and key stakeholders, while demonstrating compassionate caring behaviours to fulfill the key functions of the CMHT.
In particular, the postholder will contribute to the assessment, formulation, implementation and evaluation of programmes of care. This will require the use of a strength-based recovery-focused approach to care that proactively involves the person and optimizes self-management.
The postholder is responsible for proactively promoting partnership working with patients, carers, and other professionals/agencies in line with legislative frameworks, agreed standards and clinical policies. This involves working in a variety of home, near to home, clinic and community environments.
* Participate in performance management, personal development planning (TURAS) and clinical supervision.
* Act as mentors to nurse learners and facilitate the education and training of other disciplines.
* Participate in the team provision of a duty/response system as per rota.
* To undertake any other reasonable duties as requested by Senior Staff.
