Job summary We are looking forpart time Practice Nurse to join our team at Swanscombe Health Centre, and branch practices, who specialises/has a keen interestin Diabetes. Based at Swanscombe Health Centre, the rolewill be to work within our team, to offer support and treatment for our patients, focusing particularly on diabetes management. You should have experience of working in a similar role, with relevant qualifications, NMC registration, and have excellent organisational skills. Main duties of the job The ideal candidate will be able to perform advanced clinical skills in assessment, diagnosis and treatment, and encourage independent patient management of their diabetes. About us We have a team of over 100 staff throughout 8 sites, including GPs, Paramedic Practitioner, Clinical Pharmacists, Physicians Associates, a Physiotherapist, Practice Nurses, Healthcare Assistants, Social Prescribers and the administrative support workers. Even with a large team, we work closely together and clinical staff are supported by a weekly clinical meeting.
