They are looking for a new member for their Adult Social Worker Hospital Team. What will your responsibilities be? As an adult social worker in a hospital team, you will conduct comprehensive assessments, develop and implement care plans, provide emotional support, and coordinate resources for patients. You will assist with discharge planning, advocate for patients' rights, and maintain accurate documentation.
