* Respond to referrals for emergency and urgent assessment for individuals in crisis, assessing individuals in the most appropriate place both within the community and at A&E.
* Offer highly skilled triage and outcome advice ensuring response is appropriate to identified need.
* Provide effective specialist culturally appropriate evidence-based clinical assessment and intervention that is responsive to service priorities and service user (and carer) needs, addressing their mental health needs whilst minimising the need for hospital admission.
* Support the development of a formulation with an individual allowing them to understand the meaning of their difficulties in the context of their relationships, social circumstances, life events, and the sense that they have made of them to support recovery.
* Carry out comprehensive assessment of need for individuals in crisis using evidence-based tools to gather information and support robust formulation.
* Work within crisis model promoting a strengths-based approach service delivery which is co-produced and has the client at the centre.
* Develop crisis contingency and care plans with clients focused on meeting their identified needs.
* Undertake comprehensive risk assessment and formulate risk management plans designed to reduce and manage identified risks, co-produced with the individuals and carers.
* Formulate detailed crisis and contingency plans to minimise risk and ensure that onward referral is carried out to reflect the assessed needs. These will be co-produced.
* Promote recovery using strengths-based approach.
* Support the integration of clients into their community based on identified needs and strengths using statutory, voluntary, and peer support services as per identified need.
* Liaise and educate relatives in relation to risk and risk management where appropriate.
* In conjunction with other team practitioners, provide expert advice and support to referrers ensuring that service user and carer needs are met.
* Work closely with health colleagues, non-mental health colleagues, and a wide range of community services in order to facilitate a safe and effective plan of care.
* Work, in collaboration with general hospital staff, in facilitating effective and timely assessments and interventions for patients who might otherwise require admission to a mental health inpatient unit, or whose discharge from the acute hospital might be delayed whilst awaiting mental health assessment and allocation of appropriate mental health services.
* Ensure that all discharges from the team's caseload are effective, safe and all agreed post-discharge plans are communicated clearly to all parties involved.
* Provide teaching and education on mental health, crisis management, and risk assessment and management to non-mental health colleagues, including clients and carers.
* Support fully the concept of collaborative and co-operative multi-disciplinary/multi-agency working in the team, supporting the process of partnership working across Rotherham Doncaster and South Humber NHS Foundation Trust with partner agencies.
* Liaise effectively with other colleagues and services to ensure that safe and effective care is arranged.
* Deliver a range of evidence-based service user-centred interventions aimed at reducing distress, crisis resolution, and promoting recovery.
* Engage in assessment of self-harming behaviours and educate in relation to harm reduction and minimisation in a non-judging manner.
* Support service users to adopt recovery strategies that promote their wellbeing, healthy lifestyle, independence, and self-care.
* Refer and support engagement with identified support services.
* Rapidly and continuously evaluate the patients changing condition and response to therapeutic interventions and modify the plan of care for optimal patient outcome.
* Maintain relevant clinical and pharmaceutical knowledge, critically appraise and apply information in practice.
* Actively promote health education that may include providing alternative materials to support service users and carers in making decisions regarding their care and treatment.
* Have a working knowledge of the Mental Health Act (1983/2007) and the Mental Capacity Act (2005).
* Provide relevant support to carers and families in relation to their rights as carers and actively support their involvement in the care of the service user.
The role will involve but not be limited to you performing the above.
What you'll need:
* Registered Mental Health Nurse, Social Worker, or Occupational Therapist.
* Minimum of 18 months experience post qualifying.
* Current professional registration. Evidence of continued professional development.
* Risk assessment and management training.
* Mental health care/treatment relevant to service.
* Demonstrable experience of case managing people presenting in mental health crisis.
* Substantial experience of leading in risk assessment and risk management planning.
* Co-producing care plans and crisis plans alongside clients and families.
* Multi-agency working.
* Experience of working across service boundaries with local authorities, voluntary sector, and peer support services.
* Involving service users and carers in care delivery.
* Experience of working within a crisis service.
* Experience of acute inpatient experience.
* Working with service users and carers in service improvement.
