Job summary: We are looking for General Practice Nurse (GPN) who is motivated, independent, and interested in providing high quality clinical services to a varied population. Successful applicants may include some late shifts and weekend hours in their pattern of work. The role will include work at New Mill Street Surgery, the Extended Primary Care Service and the Lower Limb Service. The successful candidate will have a varied role working in general practice but also in the lower limb clinic in the prevention and management of leg ulcers. They will work closely with the Tissue Viability Nurse (TVN) and the Multi-Disciplinary Team (MDT) you will deliver holistic personalised care to our local population. This role also involves training and mentoring nursing, nursing associates and apprentice nurse associates, ensuring all clinical practices are compliant with the Nursing and Midwifery Council (NMC) standards. There are development opportunities which may include non-medical prescribing, clinical specialism and leadership development as part of this role. Required clinical experience: A minimum of 2 years of experience working in a primary care setting, preferably in community nursing or a similar environment. Lower Limb Management: Demonstrable experience in managing patients with lower limb conditions, particularly leg ulcers, venous disease, and related wound care. Full training in the clinic with a Tissue Viability Nurse will be offered. Able to deliver child and adult vaccinations according to national schedule. Able to perform cervical screening Responisbilities: Leg Ulcers: assessment, treatment, and prevention of leg ulcers of all aetiologies. This will include performing full lower limb assessments including ankle brachial pressure index to provide a management care plan. Venous related complications: You will also manage patients with venous eczema, lymphorrhea, wet and leaking lower limbs and other venous related lower limb complications. Oedema Management: Managing patients with chronic oedema or lymphedema, often linked with or contributing to leg ulcer development. Preventative Care: Provide education, lifestyle advice, and regular monitoring to prevent the onset of wounds. Perform, record and interpret clinical observations including blood pressure, temperature, respirations and pulse checks and manage clinically significant results appropriately following local and NICE guidance. Deliver effective and holistic management of patients with chronic diseases such as Asthma, COPD, Diabetes, Hypertension, Coronary Heart Disease and Chronic Kidney Disease. Undertake wound care/ management/dressing techniques, removal of sutures and clips. Undertake contraception assessments, provide advice and refer patients appropriately. Administer adult and paediatric vaccines under a Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs. Perform cervical screening, sexual health screening and promote breast awareness. Deliver travel healthcare, including vaccinations and advice for patients. Undertake clinical tasks including venepuncture, urinalysis and administering prescribed injections. undertake home visits in accordance with the practice policy for visits Participate in helping the practice achieve its targets and to adhere to its commitments under the quality and outcomes framework. To maintain accurate and up to date records of all patient contacts including examinations, treatment, advice and further action within the practices clinical computer system. Apply infection control measures, such as following guidelines, collecting and handling laboratory specimens, disposing of waste materials, dealing with blood and body fluid spillages and report and treat sharps injuries according to protocol. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care. Take responsibility for attending statutory / mandatory updates in accordance with organisational requirements Develop skills in relation to coaching/teaching individuals/carers/other staff and support the supervision of nursing students. Offer guidance and support to the nursing team to maintain/improve standards of care. Delegate to others, with appropriate skills, knowledge and competence.
