The Enhanced pathway is looking for an enthusiastic practitioner to work in our multidisciplinary team as a care coordinator.
The service user population is a diverse group; we work with people who are involved in mental health services for the first time and people who have longer-term and more complex needs.
Care coordination is the key role in a person's mental health journey and gives us a real opportunity to make a sustainable difference in people's lives.

Main duties of the job
The successful candidate will manage a caseload of around 25 people. They will be responsible for arranging care, liaising with other professionals and agencies, managing and reviewing care plans and risk assessments, ensuring these are done collaboratively in a recovery-focused manner. They will also ensure CPA reviews are completed in a timely manner and to a high standard, keeping the person and their carers at the center of all we do.
The team operates using the flexible assertive community treatment model, and we use a shift coordinator system on a rota. This role requires the practitioner to delegate and prioritize work, oversee and facilitate the daily morning meetings, manage the team diary, and carry out any urgent visits.
The successful candidate will be an efficient organizer, managing their workload and being flexible to meet the needs of the service. They will be supported to manage their own requirements for supervision in line with their professional registration.

JOB SUMMARY
JOB PURPOSE: The post holder will be responsible and accountable for the assessment, planning, implementation, and evaluation of skilled interventions to address identified needs and risks for allocated clients on a defined caseload. Works within a multi-disciplinary team as a member of an Enhanced CMHT.
KEY RESULT AREAS:
1. To work as a member of an integrated multi-disciplinary team providing comprehensive assessment of clients and their carers, planning, implementing, and evaluating skilled interventions to address identified needs.
2. To have knowledge of current issues and research relevant to the legislative framework and systems in which mental health care is delivered e.g. CPA, MHA (1983).
3. To contribute to service development within the clinical governance structure and policies and procedures of the Trust, adhering to NHS guidance outlined by the relevant statutory registration bodies.
Professional
1. Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.
2. To be competent in all aspects of care and risk management as defined within the framework of the care programme approach.
3. To work collaboratively with clients, carers, and multiple other agencies to provide effective care to optimize and maintain clients' mental health and overall functioning, ensuring needs are met.
