Job summary
We have an exciting opportunity for an Assistant Practitioner to support stroke survivors in Doncaster through community rehabilitation. The successful candidate will collaborate with the Multi Disciplinary Team (MDT) and manage a clinical caseload of stroke patients across various settings.
The ideal applicant will demonstrate excellent communication skills and the ability to work both collaboratively and autonomously.

Main duties of the job
This role requires a non-registered Practitioner with experience in neurorehabilitation. You will support ongoing assessment of patient needs and implement treatment plans within a multidisciplinary framework.
You must work effectively under supervision and leadership of registered professionals to deliver high-quality care to neurorehabilitation patients.
You will also provide supervision and support to Band 2 and 3 assistant staff.
Main duties include:
* Providing seamless, patient-focused care and rehabilitation in the community in Doncaster.
* Supporting the MDT in delivering high standards of therapeutic care for community stroke rehabilitation patients.
* Delivering various treatment modalities, including group exercises, education, telemedicine, and 1:1 sessions.
