Job summary Croft Medical Centre has an exciting vacancy for a Practice Nurse at our large, 14,500 patients, GMS teaching practice. We have a diverse range of nursing skills within our team providing a full complement of nursing services. Our practice also has a dispensing branch surgery in Bishops Tachbrook. As a Practice Nurse, you would be working within a friendly, committed patient centred team with excellent nursing, management and administrative support and offer a flexible working environment and regular CPD opportunities. We have a branch surgery and you will be required to work across all sites. Assess, plan and implement patient care. Evaluate the outcomes of that care, making changes to the care plan for individual patients and modifying their practice with the patients. Key Areas Of Responsibility To undertake prescribed treatments according to agreed protocols To ensure the efficient organisation of the treatment room and doctors offices, in particular: Ensuring appropriate treatment room stock and equipment levels are maintained. Routine checking of equipment to ensure correct functioning Maintaining high standards of cleanliness Carry out procedures off site when required, including vaccinations and blood sampling Administer all vaccinations and immunisations within agreed patient group directions Provide travel health advice to patients referring to current national guidance. To carry out or assist in administering emergency treatment to any patient requiring such treatment within agreed guidelines. To undertake treatment room procedures, including safe transportation principles, the safe disposal of contaminated waste. Assess, plan and implement wound care, referring to the doctors or lead nurse within agreed care protocols. Undertake administration of injectable contraceptives and offer basic family planning advice with referral to the well woman clinic or doctors as required. Administer prescribed injectable medications according to agreed protocols and patient group directions. Undertake cervical cytology screening. Carry out spirometry to assist diagnosis and care management. Carry out venepunture for screening and disease monitoring purposes Contribute to health promotion activities within the surgery, supported by the practice Health Promotion Lead Management of Long Term Conditions Plan and implement care plans for patients with long terms conditions, diabetes, heart disease, asthma and Chronic Obstructive Pulmonary Disease. Effectively review and assess care plans. treatment and management plans. Promote and support patients to make healthy lifestyle choices Participate in local and national health promotion activities. Participate in clinical audit reviews and support, and participate in local improvement projects. Record Keeping Maintain accurate patient paper and computer records according to the Nursing and Midwifery Council (NMC) Guidelines on Record Keeping (2008) Ensure patient record confidentiality at all times according to The Code of Professional Code for NMC members (2008) and maintenance of records according to the Data Protection Act (1998) Ensure practice documentation is maintained accurately and legibly Health and Safety To have knowledge of the Health and Safety at Work Act (1974) That the post holder should take reasonable actions to ensure the health and safety of themselves, fellow staff and patients. Participate in infection control audits and procedures. Personal Development and Training Will be required to participate in annual appraisal.
