Job Summary
An exciting opportunity within the Community Stroke team has arisen for either a motivated newly qualified SLT/an existing B5 SLT who is ready to progress/ or B6 SLT with a strong interest in stroke rehabilitation. The successful candidate will have a desire to work towards or have completed post-basic dysphagia training with experience of working with stroke patients and be based at Sedgefield Community Hospital. The Band 5/6 Annex 21 post will be supported by senior staff to ensure that the applicant continues to develop a broad range of specialist SLT competencies that meet the RCSLT standards. We can offer a well established network of support from peers and collaborative working within a highly skilled team. At least one year of general experience working as a speech and language therapist with completed dysphagia competencies is required to be appointed at Band 6. If you do not yet meet the essential criteria to be appointed at Band 6 you would initially be appointed at Band 5. We will provide professional development opportunities to enable career development; once the essential requirements of the Band 6 person specification are met you will be appointed to B6.
Main duties of the job
We are looking for a well-organised Speech and Language Therapist to assess, develop, and implement effective treatment plans for community-based stroke patients. Your clinical expertise will be pivotal in providing targeted therapeutic interventions with evaluations of outcomes.
Independently manage and prioritize a caseload of stroke patients.
* Collaborate closely with colleagues and relevant agencies for information exchange and attendance at weekly MDT meetings.
* Provide advice and support to patients, caregivers, and volunteers on communication and swallowing difficulties post-stroke.
* Participate in evaluating service provision, exploring different work methods, and assessing therapy outcomes.
* Supervise students and assistants and contribute to in-service training initiatives.
* Participate in regular SLT CPD and SLT clinical supervision.

The Community stroke team offers a supportive team of experienced colleagues including specialist SLT's and skilled therapy assistants allowing ready-access to support second opinions and ongoing peer supervision to deliver evidence-based practice assessment and therapy to patients across all settings.
You will help supervise speech and language therapy students, as well as contribute to the delivery of teaching programmes to a wide range of community professionals enhancing patient care.
