Job summary Visiting paramedic for Healthier West Wirral PCN Acute Visiting Service and Frailty Service. Collaborating closely with our five partner practices to provide a rapid response service to those who need it most. You will collaborate with other Healthier West Wirral PCN MDT resources, such as clinical pharmacists and social prescribers, to ensure a comprehensive approach to shared care working. Working with community teams such as palliative care, community matrons, and the district nurses to minimise patient contacts and use time more efficiently.
