Main duties of the job
Key Responsibilities:
1. Deliver individualized care based on detailed care plans.
2. Provide emotional support and maintain patient dignity.
3. Participate in clinical assessments and ensure safety protocols are followed.
4. Assist in the induction and mentoring of new team members.

Hours: 30 hours per week - Nights Only
Department: Community Services
Location: Base at St Michael's Hospice, with travel around County
Reports to: Senior Community Registered Nurse
Job Purpose: To deliver individualised, holistic palliative nursing care to patients and their families in their own homes to enhance their quality of life.
Care Provision
5. To provide individualised care to the patient and family members under the delegated direction of registered nurses, following care plans and risk assessment.
6. Following appropriate training to undertake a range of clinical care interventions stoma care.
7. To develop and utilise skills in decision-making to enhance patient care, working in an autonomous environment.
8. Contribute to clinical and health and safety risk assessments.
9. Using discretion and judgement in emergency situation.
10. To escalate changes in condition of patients/relatives to the hub senior nurse.
11. Provide emotional/psychological support to the patient and their family, identifying referrals to the wider hospice services.
12. Ensure patients privacy and dignity are maintained at all times.
13. Provide final care for the deceased patient and verification of expected death (being the extension of GPs arm)
14. Understand the complications/harm falls, pressure sores that can be caused by patient immobility and contribute to the prevention of such complications.
15. To provide medication management level one.
16. Induction of new members, sharing knowledge and skills.

20. Maintain patient confidentiality at all times.

Health and Safety
23. Act at all times to promote the safety and wellbeing of patients, relatives and carers
24. Be aware of patients that are at risk of falling and ways to prevent falls.

Education
27. Attend mandatory training and service training opportunities.
28. Develop and maintain own knowledge and skills in palliative care.
29. To undertake an appraisal and personal development review annually.
30. Act as a role model to new staff members.

31. Basic IT skills
32. In possession of a full UK driving licence, be over 21 and have held a full licence for 12 months
