This post is to support the Virtual Ward model which is a collaboration with the acute trust to deliver hospital standard treatment within a patients’ home. The virtual ward philosophy is that care is led by a specialist and provides acute clinical care at home which would otherwise be provided in a hospital for a short duration of up to 14 days. This community service is nurse led with access to support at the Virtual monitoring Hub.
This service is part of the Urgent Care Services which provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also supports hospital discharges. The service convers 24 hours a day 365 days a year.
Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end of life symptom control and management via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, as well as general observations and fundamental nursing care.
This service is providing nursing care within patients’ homes and covering West Kent. The ability to drive is essential to this post.
Band 5 Registered Nurses works closely with all users of our services and their families, friends and carers who all play an invaluable contribution in how our users experience our services. Band 5 Registered Nurses are expected to be kind and responsive but professional and informative and contribute to the quality of the services provided across the health economy by:
• Keeping the people who use our services as safe as possible through the use of sound clinical skills and effective risk assessments.
• Ensuring the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based policies and procedures.
• Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning process.
We believe that the best health care is delivered by multi-disciplinary teams working well together and in partnership with other teams and services to provide seamless care. Band 5 Nurses work as part of a team and contribute to its success by being a role model, supervising more junior members of the team and students to ensure they are working to the professional standards required and provide leadership within the team.
