An exciting opportunity has arisen for you to join our multi-disciplinary Enhanced Care Home Team.
We are looking for a registered nurse with effective communication and negotiation skills and a desire to inspire people at all levels. You will need to be innovative and be able to work autonomously, whilst providing a positive and supportive learning culture. Based at Brierley Hill Health and Social Care centre, you will work within a team made up of nurses, a dietician and occupational therapists, led by a senior nurse. The post holder will work within the Community Hub to provide leadership, education and support within care homes, with an expectation of a reduction in unnecessary, unplanned and avoidable admissions to hospital, as well as an improvement in the quality of care provision.
Main duties of the job
The post holder will work within a service that will provide leadership, education and support within care homes, with an expectation to see a reduction in unnecessary, unplanned and avoidable admissions to hospital, support safe return of residents from hospital, as well as improvement in the quality-of-care provision and improved patient experience. The post holder will assist in developing and delivering appropriate educational packages and advice to care homes on best practice and/or individual needs.
Aside from delivering education and training to care staff, you will assist in addressing specific patient needs including review, advise, or signposting of the resident/ Care Home to other services where appropriate with the overall aim of maintaining their independence in the Care Homes for as long as practicable and ultimately prevention of unnecessary hospital admissions.
The post holder will work with all disciplines and professionals to educate develop and support clear understandings of the needs and management of patients cared for within the care home environment.

Person Specification
Qualifications & Education
* Registered Physiotherapist with current HCPC registration
* Evidence of continuing professional education and training at degree level
* Post registration qualification in relevant field at specialist level or equivalent experience
* Recognised teaching qualification or equivalent experience as part of role
* Member of special interest group
* Prescriber
* Chartered Society of Physiotherapist/ British Association of Occupational Therapists
Knowlegde and Training
* Knowledge and application of principles of care for enhanced care home, long term conditions, relevant NSFs and other national standards
* Knowledge and awareness of NHS issues, current health care policy and relevance to the delivery of healthcare services
