Clinical Fellows in Enhanced Perioperative Care
Applications are invited for the post of Clinical Fellow in Enhanced Perioperative Care with opportunities in Research, Quality improvement, Clinical innovation or Education. The post will provide the opportunity to develop non-clinical skills and experience. This will also give the post-holder experience to enhance their own CV development and, along with supported clinical experience, provide additional value in their future career choices.
The aims of EPOC are to:
1. Provide higher acuity monitoring and care for the higher risk elective surgical patient, than can be provided at ward level in the early post-operative period.
2. Relieve the demand from critical care and free up their resources to care for emergency patients.
3. Prevent overnight or long stays in recovery of patients by both providing beds, but also freeing up critical care resources.
4. Improve patient outcomes and experience by both providing high acuity care, but also enabling care to be provided when originally scheduled.
EPOC is staffed by a well established and highly functional, experienced team of dedicated EPOC nurses (Bands 7,6& 5 with ~2:1 nurse: patient ratio) and clinical fellows with consultant anaesthetist / intensivist supervision for up to 48hours postoperatively. Surgical teams and allied professionals including physiotherapists, pharmacists, pain and stoma specialists are also present every day on EPOC.
Main duties of the job
1. Receive handover from nursing staff in the mornings re: any significant medical issues for patients already on EPOC.
2. Attend / conduct ward rounds.
3. Undertake clinical review of patients and clinical interventions as appropriate.
4. Be able to recognise & offer immediate clinical intervention for the deteriorating patient in line with clinical competencies & ALS protocols.
5. Be continually aware of the clinical status of patients on EPOC & be vigilant & proactive in identifying deviations from the expected post-operative course.
6. Know & act on EPOC escalation pathways when presented with issues that require senior support or when there is uncertainty regarding a course of action.
7. Present patients for senior review & complete discharge paperwork as necessary and subsequent verbal handover to parent surgical team.
8. Undertake QI - Every 3rd week is non-clinical & dedicated to QI. Fellows will be expected to record, compile & present M& M data during their time on EPOC. In addition fellows will be expected to produce at least one small audit project related to EPOC. Additional QI time may be used for preparation for applications to specialist training or other projects of the fellows choosing.
9. Attend teaching, including Thursday critical care junior doctor teaching.
10. Handover to nursing staff & surgical teams with clear plan for overnight care.
11. Provide informal ward based, learning support for resident year 5 medical students preparing for foundation year jobs.

* GMC Registration with a Full License to Practice
* MB BS (or equivalent medical qualification)
* Eligible to work at Core Level

* Formal teaching courses / qualifications
EXPERIENCE
Essential
* Demonstrable previous NHS experience or equivalent.
* ALS Provider or equivalent.
* Has achieved appropriate level of Foundation competencies.
* Experienced in Clinical Audit and Clinical Governance and the benefits of how this improves the quality of care provided to patients and ensuring this is embedded in clinical practice.
* Provide teaching to MDTs.
* Ability to teach and motivate junior medical staff.
Desirable
* Previous experience in Intensive Care Medicine, Anaesthetics or Perioperative Medicine.
Practical and Intellectual Skills
Essential
* Excellent Communication and interpersonal skills adapts style depending on audience.
* Speak fluent English to at least I.E.L.R.S level 7.5.
