It's a great time to be a part of the Community Mental Health Team (CMHT) and we are developing the Clinical Care Pathways leading to an improved and innovative service for our Service Users, Carers and Staff.

You will be based at Maidstone CMHT but may be expected to travel to our other site at Tunbridge Wells, Pembury.

Main duties of the job
The Community Mental Health Nurse is a key staff member within the multidisciplinary care team and will have continued responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change and promote health and best practice, patient safety and experience. The Mental Health Nurse will be professionally accountable to the Executive Director of Nursing and operationally accountable to the Matron / Clinical Service Manager.
Effective communication skills and self-governance are required. You will be able to demonstrate knowledge of high-level interventions & community assessment skills. Existing qualifications or a willingness to undertake training relevant to the post is expected.
The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Person Specification
Experience
* Registered on NMC
* One year experience at Band 5 or above in hospital or community health care
* Evidence of continued professional development, appropriate to role
* Clear understanding of CPA and care coordination role
* Post-registration certificate ENB 998. Possession of specialist nursing or other relevant higher academic or skills focused awards
Skills/Knowledge
* Good understanding of the principles of the Recovery Approach
* Understanding of interagency skills
* Previous experience in the field of work
