The post holder, leading a multi-disciplinary mental health care service, is responsible for ensuring that the staff operate to the requirements of the service operational policy and that safe, evidence based, effective, culturally competent care is delivered in a timely manner to service users.
They will have a leadership role in developing pathways of care and collaborative working to meet the mental health needs of service users.
The Discharge Hub is responsible for coordination of activity linked to patient flow at the Highgate West site. This management of flow includes a detailed oversight of admissions; 72 hour formulation meetings; Intended Discharge Dates; identification of barriers to discharge; Clinically Ready for Discharge cases and acute ward discharge performance. The direct team management comprises Discharge Coordinators, Senior Social Workers, and Network Navigators. The role will connect with related discharge resources including Senior Crisis Resolution Team In-Reach Nursing and Personality Disorder Liaison Nursing.
The post holder is expected to establish positive interpersonal relationships with other staff members. These relationships will include internal colleagues across the Hospital Division, across other divisions in NLFT, professional leaders, service users and carers.
There is a particular focus on driving cultural change through the Discharge Hub to amplify the voice of carers in care planning and to include VCS partners too.
External relationships are key; with local authority partners, GPs, Commissioners, accommodation pathway providers and all other services working together.
Social Worker posts within the Discharge Hub are funded by local authority partners and close working with London Borough of Camden and London Borough of Islington Heads of Mental Health Social Work is essential to ensuring statutory duties that enable effective discharge planning are delivered as expected.
The post holder will effectively lead on governance and evaluation within the service.
As an experienced leader the post holder will be expected to create an environment of continuous quality improvement and professional development.
The post holder will lead on ensuring that all team members are digitally competent and able to make good use of clinical dashboards that enable patient flow.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington.
