An exciting opportunity has arisen for a band 5 to join the SDEC and MDU teams.
This 30 hours a week post will cover both units.
Within SDEC the post holder will participate in the rapid clinical assessment of acute medical patients. The post holder will help to plan, co-ordinate, implement and evaluate care and assist in the coordination of the patient's journey in order to minimize waits, delays and prevent avoidable admissions.
The Medical Day Unit (MDU) is a Nurse Led, medical infusions, investigations and procedures unit accepting elective patients from medical specialities.
Main duties of the job
1. To maintain accurate, legible, contemporaneous records of care given.
2. Record keeping and ensuring all documentation is completed.
3. Responsibilities for ensuring patients receive high standards of care.
4. Evaluating clinical care and acting upon the outcomes of evaluation.
5. Ensuring that equipment is safe for use and report any failures in accordance with agreed department protocols.
6. Keeping up to date with clinical developments and standards eg: NICE guidance.
7. To be involved in the evaluation of clinical care and the development of action plans to address any deficiencies.
8. To maintain a portfolio of evidence of clinical skills, competency-based assessments and underpinning theoretical knowledge as required by the NMC.
9. To work in a professional autonomous manner within agreed clinical guidelines, accepted both locally and by governing bodies.
10. Receive information concerning patients' history, sensitive issues.
11. Liaise with various staff across the Trust to ensure patients' care is of a high standard.
12. To assess and carry out treatment/investigations that are required.
13. Follow pathways and protocols and evidence your work.
14. You will be expected to work as part of a team as well as independently.
