About Our Practice Our practice serves a list size of approximately 8700 patients and is supported by a dynamic and collaborative team: Thirteen GPs (plus this vacancy), three ANPs, three practice nurses, a healthcare assistant, mental health and wellbeing nurse, two practice pharmacists, a reception team that are highly skilled in signposting and an innovative practice manager
Integrated Care: Work closely with first contact physiotherapists, primary mental, social prescribers, pharmacists, paramedics, Macmillan nurses, social workers and an on-site district nurse and health visiting team.
Supportive Environment: We operate a daily huddle for all clinical staff district nurses and students which strengthens working relationships and is an excellent platform for learning. Participate in regular MDTs, palliative and cluster group meetings. Team meetings are held weekly and can be joined remotely
