Job summary
We are looking for a Practice Nurse for 24 hours per week, who has excellent clinical, communication and organisational skills, to join our existing PracticeNurse Team.
Main duties of the job
The candidate must be able to demonstrate a good understanding ofall chronic disease management including Diabetes. They must be able to undertake routine PracticeNurse duties including immunisations of both children and adults, cytology andhealth promotion.

The Practice is working closely alongside four other practices as aPrimary Care Network with a combined population of 51K. Job description
Job responsibilities
1. Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
2. Chronic disease management including Diabetes
3. Cytology
4. Routine immunizations/Childhood immunisations
5. Requesting pathology tests, for example urine culture, bloods, swabs
6. ECGs
7. Venepuncture
8. Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
9. Maintaining GP and Nurses rooms, stocking and rotating items as required
10. Chaperoning and assisting patients who are being examined by another clinician.
11. Maintain general tidiness and cleanliness of nurses and treatment rooms
12. Participation in administrative systems in the Practice
13. At all times there will be a need to maintain accurate records
14. A duty to advise senior nurses of potential problems or errors within a range of assigned tasks
15. Attend and participate in any Practice meetings when required.
16. Any other delegated duties appropriate to the post
Person Specification
Qualifications
Essential
17. Qualified and registered RGN.
18. Membership or willingness to join the RCN.
Desirable
19. Experience of working as a Nurse in a Primary Care Setting.
20. Knowledge of the Quality and Outcomes Framework.
21. Experience of using EMIS clinical system.
