An opportunity has arisen for a 4 session maternity cover GP to join our team in the Georgian market town of Farnham with good local schools and excellent transport links to London. As a practice, we are looking for a highly motivated, enthusiastic, and committed GP. We use a clinical triage platform called askmyGP.
Main duties of the job
The role will be within Farnham Park Health Group. The practice is innovative, friendly, forward-thinking, family practice, working collaboratively with other surgeries in Farnham. The successful applicant will share our ethos and commitment to delivering the best possible care to our patients. We have 4 Partners, 5 Salaried GPs, an excellent multi-skilled practice nurse team, and a list size of 17,400 as of 1st July. We utilize EMIS Web & Docman and are part of Farnham Integrated Care Services, collaborating with local practices to offer same-day access, MSK Practitioners, and working with Clinical Pharmacists and Pharmacy Technicians as part of Farnham Primary Care Network. We are consistently high QOF achievers, with workflow optimization supported by clinical practice nurse assistance.
Job responsibilities
In accordance with the Practice rota, the post-holder will undertake a variety of duties including:
1. Surgery consultations, telephone consultations, and queries
2. Visiting patients at home
3. Checking and signing repeat prescriptions
4. Dealing with queries, paperwork, and correspondence in a timely fashion
5. Making professional, autonomous decisions in relation to presenting problems
6. Assessing the health care needs of patients with undifferentiated and undiagnosed problems
7. Screening patients for disease risk factors and early signs of illness
8. Developing care plans for health in consultation with patients
9. Providing counselling and health education
10. Admitting or discharging patients to and from the caseload
11. Recording clear and contemporaneous consultation notes
12. Compiling and issuing computer-generated acute and repeat prescriptions
13. Prescribing in accordance with the Practice prescribing formulary
14. Managing NHS paperwork for own patient list plus a share of that of the other GPs
15. Proactively managing patients to ensure that Quality & Outcome Framework targets are achieved
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
