As a Support Worker, working within our Accommodation Services, you will help to promote the health and wellbeing of those you support.
This will include providing practical and emotional support; supporting people to connect with their communities, developing skills and confidence and promoting independence. We are looking for individuals with values which align to ours, who are highly motivated and committed to enriching the lives of people we support.

Shift Pattern: Rolling rota including weekends with sleep-in shifts

Job Description
The main duties of the role are to:
Under the guidance of the Service Manager
1. Be allocated a rolling rota including sleep ins. These hours will be provided at least 3 weeks in advance.
2. Take part in helping people draw up their person-centred plan and follow the agreed action plan.
3. Work in partnership with others to allow independence of choice.
4. Act with integrity and identify opportunities to celebrate achievements to allow a positive, respectful and trusting relationship to grow.
5. Support people to improve their confidence and self-esteem.
6. Support people to make informed choices around a healthy lifestyle including physical health and improving self-care.
7. Support people to manage their own physical, mental health and wellbeing in a positive manner.
8. Support people to meet their leisure, work, social, emotional and spiritual needs.
9. Support people to access the community and take part in community activities.
10. Support people to manage finances to meet domestic responsibilities and personal aspirations.
11. Provide advice and support with domestic tasks and practical issues such as dealing with neighbours and keeping their house clean.
12. Support people to access other services.
13. Support people who may have alcohol and/or drug issues.
14. Support people to maintain the safety and security of their home.
15. Prompt individuals and undertake personal care duties as required.
16. Write up and record activities undertaken.
17. Complete risk assessments in accordance with Imagine’s policy and procedure.
18. Report any significant change in an individual’s health, behaviour or wider situation to a manager and liaise with clinical teams.
19. Complete sleep over shifts as part of an agreed rota (sleep overs are allocated in advance).
Person Specification
1. NVQ Level 2 in Health & Social Care/care certificate desirable or equivalent.
2. Experience of working in a community care setting is desirable.
3. Commitment to supporting people consistent with their rights, choices and responsibilities.
4. Demonstrate a positive attitude to people with mental health problems.
5. Ability to provide sympathetic, emotional and practical support to tenants.
6. Ability to support tenants to make informed choices within their daily lives.
7. An understanding of how to promote inclusion for people with mental health needs at all levels.
8. Domestic skills relating to general household tasks (cooking, cleaning, shopping and managing money).
9. Knowledge of safety issues in a domestic setting.
10. Creative problem-solving skills.
11. Excellent verbal and written communication skills, paying attention to detail, whilst being able to deliver high quality, accurate work quickly.
12. Understanding of person-centred working.
13. Working with and relating to people from a wide variety of backgrounds.
14. Working as a member of a team.
15. Reliable and trustworthy.
16. Non-judgmental.
17. Enjoys a challenge.
18. Sensitive & approachable.
19. Willingness to attend training courses and events.
