Suffolk andNorth East Essex ICB (SNEE ICB) have exciting opportunities for substantiveposts for Registered Nurses (Adult, Mental health or Learning Disability) to coordinate NHS Continuing Healthcare assessments, casemanage individuals who are eligible for NHS Continuing Healthcare and supportthe Discharge to Assess model. We have a number of roles availablewithin the NHS Continuing Healthcare team which offer varied and interestingwork, including undertaking NHS Continuing Healthcare assessments for people athome, in care homes and virtually, supporting personalisation through the useof Personal Health Budgets for individuals who are eligible for NHS ContinuingHealthcare and wish to manage their own care package, and commissioning andcase management of placements and care packages for individuals eligible forNHS Continuing Healthcare funding.

Main duties of the job
Responsible for coordinatingand carrying out comprehensive assessments and making eligibilityrecommendations using the appropriate local and national tools as defined inthe guidance for NHS CHC and NHS-FNC.
To collate data andevidence from comprehensive assessments to form reports.
Responsible forcompletion and review of person-centred, outcome-focussed care and supportplans for individuals deemed eligible for NHS funding, ensuring that the agreedpackage of care and support meet the individuals assessed needs.
To support individualsto access Personal Health Budgets to manage their care and support.
To carry out reviews ofcare and support and eligibility for NHS funding where required.
To support with thelocal resolution process where eligibility decisions have been challenged.
To carry out MentalCapacity Assessments and make Best Interests Decisions as appropriate, in linewith relevant legislation.
To support with and beinvolved in the process of making applications to the Court of Protection orResponsible Body for authorisation of Deprivation of Liberty, includingconsideration of the necessity and proportionality of proposed deprivations.
To provide clinicaladvice and support to care homes and home care providers.

Job responsibilities
Asan NHS CHC Nurse Coordinator you will provide quality, evidence basedassessments, nursing advice and support primarily for service users from theage of 18 to determine eligibility for NHS CHC and NHS Funded Nursing Care(FNC) ensuring their needs are identified and met in accordance with theNational Framework for NHS CHC and NHS FNC.
CaseManagement is an intrinsic function of the role with the aim of supportingindividuals with complex needs and their families/carers in accessing the mostappropriate care to meet their identified needs, including supporting withPersonal Health Budgets (PHBs) and preparation of person-centred care andsupport plans. Case management will focus on ensuring all commissioned packagesof care are safe, of high quality, meet identified outcomes, and represent bestvalue for money in line with the ICBs Equity and Choice Policy.

Youwill work closely with colleagues in other organisations such as the acutehospitals, community services, care providers and voluntary organisations, andwill build professional relationships to improve patient experience.

You are required to be a car driver andhave access to a car as you will regularly be assessing individuals across SNEEand occasionally out of team covers the whole of SNEE, however, we our assessmentand review functions work in localities, working flexibly to cover across thepatch where needed.
The team has a focus on continuousimprovement to ensure we deliver high standards and work productively to ensurerobust and timely decision making for our patients.
Previous experience of NHS ContinuingHealthcare is not essential however, you should have experience in assessingindividuals with complex health and social care needs, including coordinatingand planning care. You will be an excellent communicator with soundinterpersonal skills. You will be expected to have good IT skills,including Microsoft Word and be experienced in using a clinical system forrecording patient information. You will need to have the ability to manageemotive and sensitive issues with tact and diplomacy, work well under pressure,negotiate with confidence and maintain a high level of professionalism andproductivity.

16. Awareness of National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and its implementation.
17. Demonstrable experience and knowledge of current key policies and priorities within the NHS and Social Care.
18. An understanding of the Mental Capacity Act 2005 and its application.
19. Good working knowledge of Adult Safeguarding
