We are seeking to recruit a full-time Band 6 registered nurse to the post of Enhanced Care Practitioner, to manage and support our team of health care support workers as part of our Enhanced Care Team, who provide 1:1 support and meaningful activity/intervention for in-patients who have enhanced care needs. This includes patients who require specialist mental health support while receiving care in the acute general hospital environment.
This is a 6-month fixed-term post, and is an exciting opportunity for someone who wishes to develop and progress their management skills.

2. Contribute towards sharing good practice within the team and multidisciplinary team.
3. Promote a positive image of the Trust at all times.
4. Work cohesively with all members of the multidisciplinary team and promote effective liaison with the team in ensuring that the very best services to patients are provided at all times.
5. Communicate effectively, motivating and supporting multidisciplinary team members in situations that may be sensitive and complex in nature, always maintaining patient confidentiality.
6. To be able to communicate effectively and compassionately with patients, their families, and carers.
