Primary Responsibilities:
Clinical Practice:
* In accordance with the practice rota, the post-holder will undertake duties including GP Face to Face consultations, telephone consultations, home visits, checking and signing repeat prescriptions, and dealing with queries in a timely manner.
* Making professional, autonomous decisions in relation to presenting problems.
* Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
* Screening patients for disease risk factors and early signs of illness.
* Developing care plans in consultation with patients and in line with current practice disease management protocols.
* Providing counselling and health education.
* Referring to other care providers as appropriate.
* Recording clear and contemporaneous consultation notes to agreed standards.
* Collecting data for audit purposes.
* Issuing acute and repeat prescriptions in accordance with the practice prescribing formulary.
* Visiting patients during surgery time in an emergency if clinically indicated.
* Undertaking a fair share of home visiting sessions per week as allocated.
Communication:
* Utilise sensitive communication styles to ensure patients are informed and consent to treatment.
* Communicate with and support patients receiving ‘bad news’.
* Recognise the need for alternative methods of communication to overcome different levels of understanding and cultural backgrounds.
Delivering a Quality Service:
* Work within own competence and professional code of conduct as regulated by the GMC.
* Produce accurate records of patient consultations consistent with legislation and policies.
* Assess effectiveness of care delivery through self and peer review.
* Evaluate patients’ response to healthcare provision.
* Support shared learning across the practice.
* Use structured frameworks to manage and review patient complaints and clinical incidents.
Leadership – Personal and People Development:
* Act as a GP leader, ensuring patient needs are a priority.
* Take responsibility for own learning and performance.
* Support staff development to maximize potential.
* Work with management to ensure adequate staff quality and skill-mix.
* Contribute to the development of local guidelines and protocols.

* Understand own role and scope and identify development opportunities.
* Work as an effective team member, supporting others.

* Create clear referral mechanisms to meet patient needs.
Management of Risk:
* Manage and assess risk within areas of responsibility.
* Support the team to ensure safe work areas conform to health and safety legislation.
* Ensure appropriate supervision of vaccine and drug storage and documentation.

* Use technology to aid management in planning and monitoring care.
* Review and process data using accurate Read codes for easy retrieval.
* Understand responsibilities regarding the Freedom of Information Act.
Equity and Diversity:
* Identify and act against discrimination to promote diversity and equality.
* Support others in promoting equality in a non-discriminatory culture.
* Assist patients from marginalized groups to access quality care.
Qualifications:
Essential:
* Qualified GP (Newly Qualified General Practitioner with under 2 years from qualification).
* MRCGP.
* Vocational Training Certificate or equivalent.
* General Practitioner (Certificate of Completion of Training CCT).
* Understanding of evidence-based practice and NICE guidelines.
Desirable:
* Knowledge of local public health issues.
* Able to identify health determinants in the area.
* Awareness of issues within the wider health arena.
Eligibility:
* Full GMC registration.
* National Performers List registration (England).
* Appropriate defence indemnity (MPS/MDU).
* Evidence of current validation and last appraisal.
* Eligibility to practice in the UK independently.
Experience:
* Experience in a primary care environment.
* Experience of continued professional development.
* Experience of QOF and clinical audit.
* Experience of medicines management.

Pay: Up to £73,113.00 per year
