An exciting opportunity for a Clinical Pharmacist has arisen to join our Walnut Medical Centre Practice. PREVIOUS EXPERIENCE IN GENERAL PRACTICE IS ESSENTIAL. There may be room for negotiation on the terms and salary depending on experience. The post is fully based at Walnut Medical Centre although remote working for some sessions is possible. The successful applicants will work closely with the existing clinical teams to deliver patient care working innovatively and collaboratively.
The number of hours is flexible and may be shared with more than one applicant.

Completion of CPPE and Independent Prescriber qualifications are an advantage and would be preferred.

Main duties of the job
A unique opportunity has arisen to work alongside our current team and pharmacists to join the Team. We are looking for a dynamic, enthusiastic Pharmacist to support and deliver our vision, mission and values. You will deliver expertise in clinical pharmacy and provide Clinical Pharmacy input is required to the teams based at Walnut Medical Centre. You will be assisting in the delivery of the practice vision and collaborating across multidisciplinary networks to promote new ways of working to deliver the best possible care to the local population.
Job responsibilities
The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy. Additionally, to engage with medicines optimisation, MHRA, medication reviews and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services. You will also help in the management of patients and informing them of relevant investigations and actions.
Responsibilities and Duties:
1. See (where appropriate) patients with single or multiple medical problems where medicine optimisation is required (e.g. COPD, asthma).
2. Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking ensuring they get the best use of their medicines (i.e. medicines optimisation). Make appropriate recommendations to GPs for medicine improvement.
3. Patient facing clinical medication review and relaying of results and actions.
4. Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.
5. Management of common/minor/self-limiting ailments while working within a scope of practice and limits of competence.
6. Provide patient facing clinics for those with questions, queries and concerns about their medicines in the practice.
7. Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
8. To reconcile medicines following discharge from hospitals, intermediate care and into care homes.
9. Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time.
10. Produce and implement a practice repeat prescribing policy.
11. Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Qualifications
* MUST HAVE EXPERIENCE OF WORKING WITHIN GENERAL PRACTICE
* Degree in Pharmacy, MPharm/BPharm
* GPhC Registered
* Full driving licence
* CPPE
* IP Qualified or working toward an IP qualification
* General Practice and understanding of medicines optimisation roles
